Factores asociados al desarrollo de lesión renal aguda en pacientes con COVID-19 en el Hospital Regional de Loreto, Perú año 2021

Abstract

Objective: To determine the factors associated with the development of acute kidney injury in patients with COVID-19 at the Regional Hospital, Peru 2021. Materials and methods: Retrospective case-control analytical design study (118 cases and 236 controls) made up of all patients with a diagnosis of COVID-19 confirmed by molecular or antigen testing hospitalized at the Loreto Regional Hospital from January to June year 2021. A data collection form was used to collect the information obtained in this study. Results: Risk factors associated with the development of kidney injury were older than 60 years (OR: 1,647; IC del 95%: 1,044 - 2,600; p: 0,032), origin from a rural area (OR: 2,151, IC del 95%: 1,153 - 4,013; p: 0,014), hospitalization time greater than 10 days (OR: 2,299; IC del 95%: 1,419 - 3,723; p: 0,001), ICU admission (OR: 6,393; IC del 95%: 3,781 - 10,810; p: 0,000), arterial hypertension (OR: 2,497; IC 95%: 1,571 - 3,968; p: 0,000), diabetes mellitus (OR: 2,627; IC 95%: 1,566 - 4,408; p: 0,000), obesity (OR: 2,322, IC del 95%: 0,991 - 5,442; p: 0,047), chronic liver disease (OR: 3,173, IC del 95%: 1,100 - 9,149; p: 0,025), chronic kidney disease (OR: 5,185, IC del 95%: 0,990 - 27,157; p: 0,031) and the use of mechanical ventilation (OR: 6,393; IC 95%: 3,781 - 10,810; p: 0,000). The rest of the variables do not present statistical significance. Conclusions: The factors associated with the development of acute kidney injury were being older than 60 years, rural origin, hospitalization time longer than 10 days, ICU admission, high blood pressure, diabetes mellitus, obesity, chronic liver disease, chronic kidney disease and the use of mechanical ventilation.

Objetivo: Determinar los factores asociados al desarrollo de lesión renal aguda en pacientes con COVID-19 en el Hospital Regional, Perú 2021. Materiales y métodos: Estudio de tipo analítico de diseño Caso-Control retrospectivo (118 casos y 236 controles) conformado por todos los pacientes con diagnóstico de COVID-19 confirmado por prueba molecular o antígena hospitalizados en el Hospital Regional de Loreto de Enero a Junio del año 2021. Se utilizó una ficha de recolección de datos para la recopilación de la información obtenida en este estudio. Resultados: Se obtuvo como factores de riesgo asociado al desarrollo de lesión renal ser mayor de 60 años (OR: 1,647; IC del 95%: 1,044 - 2,600; p: 0,032), procedencia de zona rural (OR: 2,151, IC del 95%: 1,153 - 4,013; p: 0,014), tiempo de hospitalización mayor de 10 días (OR: 2,299; IC del 95%: 1,419 - 3,723; p: 0,001), ingreso a UCI (OR: 6,393; IC del 95%: 3,781 - 10,810; p: 0,000), hipertensión arterial (OR: 2,497; IC 95%: 1,571 - 3,968; p: 0,000), diabetes mellitus (OR: 2,627; IC 95%: 1,566 - 4,408; p: 0,000), obesidad (OR: 2,322, IC del 95%: 0,991 - 5,442; p: 0,047), enfermedad hepática crónica (OR: 3,173, IC del 95%: 1,100 - 9,149; p: 0,025), enfermedad renal crónica (OR: 5,185, IC del 95%: 0,990 - 27,157; p: 0,031) y el uso de ventilación mecánica (OR: 6,393; IC 95%: 3,781 - 10,810; p: 0,000). El resto de variables no presentan significancia estadística. Conclusiones: Los factores asociados al desarrollo de lesión renal aguda fueron ser mayor de 60 años, procedencia rural, tiempo de hospitalización mayor de 10 días, ingreso a UCI, hipertensión arterial, diabetes mellitus, la obesidad, enfermedad hepática crónica, enfermedad renal crónica y el uso de ventilación mecánica.
