Adherencia terapéutica en pacientes con insuficiencia cardíaca congestiva e hipertensión arterial primaria atendidos en un hospital de Iquitos 2024

Abstract

Congestive heart failure (CHF) is a chronic condition in which the heart does not pump enough blood, leading to congenital heart disease and sudden cardiac death. Primary arterial hypertension (PAH) is a chronic condition characterized by blood pressure levels above 140/90 mmHg. The objectives of this study were to identify sociodemographic aspects, medication-related problems, and therapeutic adherence using the Morisky Green Test in patients with CHF and PAH treated at the Internal Medicine Service of Hospital III – Iquitos, EsSalud, from November 2024 to May 2025. The study employed a quantitative, prospective, descriptive, cross-sectional, and quasi-experimental design, allowing for statistical analysis of the data. The study population consisted of 35 patients with primary arterial hypertension (PAH) and 25 patients with congestive heart failure (CHF). The methodology used was pharmacotherapeutic monitoring to determine drug-related problems (DRPs), using the Second Granada Consensus, and treatment adherence was assessed using the Morisky Green test. The results obtained for both diseases showed a higher prevalence in the 60-70 age group, residing in the Iquitos district, and married individuals in both conditions. Females were more predominant (69.0%), with DRP5 at 48.9% and therapeutic adherence at 77.1% for hypertension (HTPA). For congestive heart failure (ICC), males were more predominant (68.0%), with DRP5 at 37.5% and therapeutic adherence at 72.0%. In conclusion, effective pharmacotherapeutic monitoring improved therapeutic adherence in both pathologies, reducing the main symptoms and improving quality of life.
La Insuficiencia cardiaca congestiva (ICC) es una afección crónica que ocasiona que el corazón no bombea la cantidad suficiente de sangre produciendo la cardiopatía congénita y muerte repentina cardiaca, la Hipertensión arterial primaria (HTAP) es una enfermedad crónica, cuando los niveles de la presión arterial esta sobre 140/90 mm de Hg. Los objetivos fueron: Identificar los aspectos sociodemográficos, problema relacionado con medicamentos y la adherencia terapéutica con el Test de Morisky Green en pacientes con ICC e HTAP, atendidos en el Servicio de Medicina Interna del Hospital III – Iquitos, EsSalud, durante el mes de noviembre del 2024 hasta mayo 2025, el estudio se llevó a cabo bajo un enfoque cuantitativo, diseño prospectivo, descriptivo, transversal y cuasiexperimental, que permitió el tratamiento estadístico de la información. La población fue de 35 pacientes con Hipertensión arterial primaria (HTAP) y 25 pacientes con Insuficiencia cardiaca congestiva (ICC). Como metodología se utilizó el seguimiento farmacoterapéutico para determinar los PRMs, utilizando Segundo Consenso de Granada y así decretar la adherencia al tratamiento mediante el test de Morisky Green. Los resultados obtenidos en ambas enfermedades fueron con mayor prevalencia la edad de 60 a 70 años, Distrito de Iquitos, condición civil fue casados en ambas enfermedades. En el sexo fue más predominante sexo femenino con 69,0%, PRM5 con 48,9 % y adherencia terapéutica con 77,1% para HTPA, para ICC, sexo masculino con 68,0%, PRM5 37,5% y adherencia terapéutica con 72,0%. Se concluye, mediante seguimiento farmacoterapéutico eficaz se mejoró la adherencia terapéutica en ambas patologías, disminuyendo los síntomas principales y mejorando la calidad de vida.
