Relación del índice neutrófilo/linfocito con severidad de pancreatitis en pacientes hospitalizados en el Hospital Regional de Loreto 2023 – 2024

Abstract

Objective: To evaluate the relationship between the neutrophil-to-lymphocyte ratio (NLR) and the severity of acute pancreatitis in patients hospitalized at the Regional Hospital of Loreto during the period 2023-2024. Methods: Observational, cross-sectional, retrospective study with a diagnostic test approach. A total of 105 patients diagnosed with acute pancreatitis were included. The association between NLR and severity was evaluated using the Mann-Whitney test. The area under the ROC curve, the optimal cutoff point using the Youden index, as well as sensitivity, specificity, and predictive values were analyzed. Results: Severe acute pancreatitis was observed in 6.7 % of patients. NLR values were significantly higher in severe cases [23.09 (10.32-62.70)] compared to non-severe cases (p = 0.006). NLR showed good discriminative ability with an AUC of 0.812 (95 % CI: 0.673-0.950). The optimal cut-off point was 8.43, with 100 % sensitivity and 55.1 % specificity. The positive predictive value was 13.73 % and the negative predictive value was 100%. Conclusion: The neutrophil-to-lymphocyte ratio was significantly associated with the severity of acute pancreatitis and represents a useful tool to rule out severe cases due to its high sensitivity and high negative predictive value.

Objetivo: Evaluar la relación entre el índice neutrófilo-linfocito (INL) y la severidad de la pancreatitis aguda en pacientes hospitalizados en el Hospital Regional de Loreto durante el periodo 2023-2024. Métodos: Estudio observacional, transversal y retrospectivo, con enfoque de pruebas diagnósticas. Se incluyeron 105 pacientes con diagnóstico de pancreatitis aguda. La asociación entre el INL y la severidad se evaluó usando la prueba de Mann-Whitney. Se analizó el área bajo la curva ROC, el punto de corte óptimo mediante el índice de Youden, así como la sensibilidad, especificidad y valores predictivos. Resultados: El 6.7 % de los pacientes presentó pancreatitis aguda grave. Los valores del INL fueron significativamente mayores en los pacientes con formas graves [23.09 (10.32-62.70)] en comparación con los no graves (p = 0.006). El INL mostró una adecuada capacidad discriminativa con un AUC de 0.812 (IC 95 %: 0.673-0.950). El punto de corte óptimo fue de 8.43, con una sensibilidad del 100 % y especificidad del 55.1 %. El valor predictivo positivo fue de 13.73 % y el valor predictivo negativo del 100 %. Conclusión: El INL se asoció significativamente con la severidad de la pancreatitis aguda y constituye una herramienta útil para descartar formas graves, debido a su alta sensibilidad y elevado valor predictivo negativo.
