Efectividad del índice de El-Ganzouri como predictor de intubación traqueal difícil en anestesia general de pacientes adultos en el Hospital Regional de Loreto, 2019-2024

Abstract

The American Society of Anesthesiologists (ASA) defines a difficult airway (DA) as the presence of clinical conditions that make both mask ventilation and endotracheal intubation difficult in patients under these circumstances. This situation occurs in 0.05% to 0.1% of procedures. (2) On the other hand, difficult tracheal intubation (DTI) refers to cases that exceed three attempts or take more than ten minutes to achieve successful intubation. (1-4) Preoperative prediction of difficult tracheal intubation (DTI) is a critical component of anesthetic assessment. This is because the existence of an unanticipated complicated airway is associated with a significant increase in perioperative morbidity and mortality. In this context, having clinical tests that are simple, reproducible, quick to perform, and highly diagnostic is essential for safe anesthetic practice (8). The objective of this study is to analyze the value of the El-Ganzouri index for the early detection of cases of complex tracheal intubation in adults undergoing general anesthesia at the Felipe Santiago Arriola Iglesias Regional Hospital in Loreto between 2019 and 2024. The methodology applied for this study followed an observational, analytical, cross-sectional, and retrospective approach.La American Society of Anesthesiologists (ASA) da como concepto de la Vía Aérea Difícil (VAD) a la presencia de condiciones clínicas que dificultan tanto la respiración asistida con mascarilla como la intubación endotraqueal en pacientes que se encuentran bajo estas circunstancias. Esta situación se presenta en un rango de 0.05% a 0.1% de los procedimientos. (2), Por otro lado, la intubación traqueal difícil (ITD) se refiere a los casos que superan los tres intentos o tardar más de diez minutos en conseguir la intubación con éxito. (1-4). La predicción preoperatoria de una intubación traqueal difícil (ITD) es un componente crítico en la evaluación anestésica, Esto se debe a que la existencia de una vía aérea complicada que no haya sido anticipada se vincula con un incremento significativo en la morbimortalidad perioperatoria. En este contexto, contar con pruebas clínicas que sean simples, reproducibles, rápidas de ejecutar y con un alto rendimiento diagnóstico resulta fundamental para la práctica anestésica segura (8). Este estudio tiene como objetivo analizar el valor del índice de El-Ganzouri para detectar de manera anticipada casos de intubación traqueal compleja en adultos sometidos a anestesia general en el Hospital Regional de Loreto “Felipe Santiago Arriola Iglesias”, en el periodo 2019-2024. La metodología aplicada para este estudio siguió un marco observacional, enfoque analítico, transversal y retrospectivo.
