Crecimiento y desarrollo neurocognitivo de niños y adolescentes con infección por VIH en la ciudad de Iquitos, 2025

Abstract

Objective: To determine the relationship between HIV infection and physical growth and neurocognitive development in children and adolescents in the city of Iquitos. Methods: A case–control study was conducted at the Hospital Regional de Loreto and Hospital Apoyo Iquitos, evaluating 21 children and adolescents with HIV and 67 healthy controls from the same age range and locality. Anthropometric measurements and a standardized battery of neurocognitive tests were administered. Caregiver sociodemographic characteristics and antiretroviral therapy (ART) adherence were recorded for associative analysis. Results were obtained through descriptive analysis and inferential statistical tests (Student’s t-test, ANOVA, and correlations analyses) to compare groups and assess associations. Results: The study included 88 participants. Children and adolescents living with HIV presented lower weight and height (p < 0.001), with reduced height-for-age Z scores, interpreted as growth delay. They also showed lower neurocognitive performance in attention (p = 0.02), processing speed (p = 0.01), working memory (p = 0.01), and semantic fluency (p = 0.02). ART adherence was associated with better neurocognitive outcomes (IFS p = 0.009) and higher height-for-age (p < 0.001). Additionally, caregiver educational level, age and type showed a significant relationship with several neurocognitive domains (r = 0.26–0.84). These findings highlight the interaction between clinical and family factors in the physical growth and neurocognitive development of this population. Conclusion: Children and adolescents living with HIV in Iquitos exhibit selective impairments in physical growth and neurocognitive development, influenced by treatment adherence and caregiver educational support.

Objetivo: Determinar la relación entre la infección por VIH y, el crecimiento físico y el desarrollo neurocognitivo en niños y adolescentes de la ciudad de Iquitos. Métodos: Estudio de casos y controles, realizado en Hospital Regional de Loreto y Hospital Apoyo Iquitos que evaluó a 21 niños y adolescentes con VIH y 67 controles sanos del mismo rango etario y localidad. Se aplicaron mediciones antropométricas y una batería estandarizada de pruebas neurocognitivas; se registraron características sociodemográficas del cuidador y el nivel de adherencia al TAR para su análisis asociativo. Los resultados se obtuvieron mediante análisis descriptivo y pruebas inferenciales (t de Student, ANOVA y correlaciones) para comparar grupos y evaluar asociaciones. Resultados: En el estudio se incluyó 88 participantes. Los niños y adolescentes con VIH presentaron menor peso y talla (p < 0.001), con puntajes Z talla/edad disminuidos, interpretándose como retraso del crecimiento. También presentaron desempeños neurocognitivos inferiores en atención (p = 0.02), velocidad de procesamiento (p = 0.01), memoria de trabajo (p = 0.01) y fluidez semántica (p = 0.02). La adherencia al TAR se asoció con mejores resultados neurocognitivos (IFS p = 0.009) y mayor talla para edad (p = ˂0.001). Asimismo, el nivel educativo, edad y tipo de cuidador mostró una relación significativa con varios dominios neurocognitivos (r = 0.26–0.84). Estos hallazgos resaltan la existencia de interacción entre factores clínicos y familiares en el crecimiento físico y desarrollo neurocognitivo de esta población. Conclusión: Los menores de edad con VIH en Iquitos muestran afectaciones selectivas en crecimiento físico y desarrollo neurocognitivo, influenciadas por la adherencia al tratamiento y el apoyo educativo del cuidador.
