Dimorfismo sexual y cuarto conducto del primer molar mandibular permanente con tomografía cone beam en consultorio de la práctica privada, Iquitos–2024

Abstract

The objective of this study was to investigate the potential association between sexual dimorphism and the presence of a fourth canal in the permanent mandibular first molar (CCPMP). The sample consisted of 359 patients 204 females (56.8%) and 155 males (43.2%) who underwent cone-beam computed tomography (CBCT). The scans were obtained from the digital archive of a private dental practice in the city of Iquitos, Loreto, between 2018 and 2024. All tomographic images that met the established inclusion and exclusion criteria were included in the study. Axial, sagittal, and coronal tomographic sections were analyzed to assess the presence of the CCPMP, as well as the number of foramina and roots in the distal portion of the mandibular first molar. Statistical analysis was performed using SPSS version 27, applying Pearson’s Chi-square test with a significance level set at p < 0.05. The mean age of the study sample was 38.89 ± 16.28 years. The overall prevalence of the CCPMP was 16.7%, with 17.2% observed in females (35 of 204) and 16.1% in males (25 of 155), showing no statistically significant association with sex (p = 0.796). It is therefore concluded that no relationship exists between sexual dimorphism and the presence of a fourth canal in the permanent mandibular first molar.

El propósito de esta investigación fue explorar la posible asociación entre el dimorfismo sexual y la existencia de un cuarto conducto en el primer molar mandibular permanente (CCPMP). La muestra estuvo conformada por 359 pacientes, 204 mujeres (56.8%) y 155 hombres (43.2%) a los que se les realizó un estudio con tomografía cone beam, las tomografías fueron obtenidas de un archivo virtual de un consultorio de la práctica privada en la ciudad de Iquitos - Loreto, entre los años 2018 y 2024, se incluyeron todas las tomografías que cumplieron los criterios de inclusión/exclusión. Se realizaron cortes tomográficos en planos axiales, sagitales y frontales, con el fin de examinar la existencia del CCPMP, así como la cantidad de forámenes y raíces presentes en la porción distal del primer molar inferior. El análisis estadístico fue llevado a cabo utilizando el software SPSS en su versión 27, aplicando la prueba de Chi-cuadrado de Pearson con un nivel de significancia de p < 0.05. La edad promedio de la muestra estudiada fue 38.89 ± 16.28 años de ellos la prevalencia del CCPMP fue del 16,7%, el 17.2% fueron mujeres (35 de 204) y el 16.1% fueron hombres (25 de 155) no existiendo relación estadísticamente significativa según sexo (p=0.796). Se concluye que no hay relación entre el dimorfismo sexual y la presencia del cuarto conducto de la primera molar mandibular permanente.
